Responsibilities

• Follow physician-prescribed care plans.
• Administer medications and treatments.
• Conduct patient assessments and monitor vitals.
• Educate patients and families on care.
• Collaborate with CNAs, HHAs, and Case Managers.
• Document care accurately and in a timely manner.
• Perform activities of daily living as needed.
