Responsibilities

• Execute physician-prescribed plans of care.
• Document care provided in the system of record.
• Administer prescribed medication, treatment, and therapies.
• Assist patients with activities of daily living.
• Monitor changes in patient condition and report.
• Communicate with family, physicians, and case managers.
• Provide one-on-one in-home care and supervise CNAs/PCAs.
• Perform other personal care services as needed.
