Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license and access to a dependable vehicle.
• Ability to function in any home situation regardless of client characteristics.

Responsibilities

• Provide high-quality clinical services within scope of practice.
• Coordinate care with other members of the patient/client's care team.
• Complete clinical nursing assessments as required.
• Develop and revise individualized plans of care with community providers.
• Ensure plan of care frequency meets patient needs.
