Job summary

Select Home Health is seeking a skilled and compassionate Registered Nurse to provide in-home nursing care in McCordsville, IN. You will assess patients, implement care plans, and document OASIS for multiple patients daily.

Educational requirements include RN licensure and at least 1 year of nursing experience; valid driver’s license and TB test are required. You will educate patients and caregivers, coordinate with physicians, and supervise LPNs/HHAs as needed.

Qualifications

Graduate of an accredited school of professional nursing.
Current RN license for the practicing state.
Minimum of 1 year nursing experience in a hospital, facility, or home health setting; home health experience preferred.
Valid driver's license, reliable auto insurance, and personal transportation.
Current up to date TB test.

Responsibilities

Perform patient assessments and ongoing evaluations.
Implement physician-ordered plans of care and complete required documentation (OASIS).
Provide in-home nursing care for multiple patients per day.
Educate patients and caregivers on medications, disease processes, and in-home care.
Coordinate patient services and supervise LPNs/HHAs as needed.
Communicate changes in patient condition to physicians and care teams.
Maintain compliance with regulations and agency policies.
