Job summary

Qualifications

Responsibilities

• Assess and chart patient observations at each visit.
• Perform accurate assessments within defined time frames.
• Document problems with patient, family, and team collaboration.
• Review medications and vital signs during visits.
• Administer physician-prescribed medications.
• Update care plans to meet outcomes.
• Educate patients and families on home health procedures.
• Coordinate with Physicians, Therapists, and care team.
• Maintain HIPAA confidentiality.
• Travel between locations in personal vehicle.
