Job summary

Parx Home Health Care is seeking a Registered Nurse to join our in-home care team in Florida. You will assess patients, create individualized care plans, educate families, and coordinate with physicians and other providers to ensure seamless care in patients' homes.

The role requires an active Florida RN license, 1–3 years in home health or related settings, and strong communication skills. A valid driver’s license and CPR certification are required, with clean background checks and TB testing

Qualifications

Active Florida RN license
1–3 years of clinical experience in home health, hospice, or community health
CPR certification required
Level 2 criminal background check
Valid Florida driver’s license and reliable transportation
Two-Step TB test or Chest X-ray (within 6 months)

Responsibilities

Conduct comprehensive assessments of patients’ health status, including physical, emotional, and social factors.
Develop and implement individualized care plans tailored to each patient’s needs and goals.
Provide patient education to promote self-management and prevent complications.
Coordinate care with physicians, therapists, and social workers to ensure continuity of care.
Administer medications, treatments, and procedures as ordered by the physician or authorized provider.
Document all patient care activities and communicate progress to the healthcare team.
Participate in quality improvement activities to enhance patient outcomes.
Maintain knowledge by attending continuing education and staying current with nursing developments.
Provide compassionate care to patients and families while adhering to laws and ethical standards of nursing practice
