You will collaborate with an interdisciplinary team to support whole-person care—mind, body, and spirit—while contributing to meaningful community impact and professional growth.

Qualifications

Responsibilities

• Deliver in-home rehabilitation under the direction of a licensed Physical Therapist.
• Implement individualized care plans focusing on mobility, strength, and functional independence.
• Document progress and communicate with patients, caregivers, and team members.
