Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to deliver patient-centered care in patients' homes, under the guidance of a Physical Therapist. The role emphasizes functional improvement, collaboration with healthcare teams, and maintaining comprehensive documentation.

Requirements include a current Physical Therapy license, valid driver's license with insurance, CPR certification, and at least 1 year of PTA experience.

Qualifications

• Current state license to practice Physical Therapy.
• Valid driver's license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Comprehensive knowledge of geriatric physical, emotional, social and biological changes.
• Read, write, and speak fluent English.
• One year of experience as a Physical Therapy Assistant.

Responsibilities

• Administer the physical therapy treatment plan to patients, working onsite in their homes.
• Administer non-complex manual therapeutic exercises, massage, traction heat, light, cold, water, and electrical modalities.
• Document client records and submit clinical documentation weekly.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment failures, supply issues, or staff/client changes.
• Apply infection control and universal precautions in patient care activities.
