Job summary

FOX Rehabilitation is seeking licensed Physical Therapist Assistants in Rockford, IL to deliver one-on-one, in-home care for older adults. You’ll work with a supervising PT to implement individualized treatment plans and guide patients through exercises and functional activities in their homes and communities.

Qualifications

• Valid Physical Therapist Assistant license in the state(s) of practice, or eligibility to obtain
• Graduate of an accredited Physical Therapist Assistant program
• Current CPR certification
• Basic computer skills; proficient in Microsoft Office

Responsibilities

• Deliver individualized treatment plans established by supervising PT
• Instruct, motivate, and safeguard patients through therapeutic exercises and activities
• Document visits accurately in FOX's EMR system per clinical standards
• Coordinate care with patients, POAs, and referral sources
