Job summary

In Home Rehab of Florida seeks a compassionate Physical Therapist Assistant to join our Palm Beach County team. You will work one-on-one with older adults in the home setting, building consistent visits through multiple agency contracts.

Active Florida PTA license is required; prior home health or geriatric experience is preferred. The role offers a flexible schedule, competitive pay, and strong team support to help you grow in a home-health-focused practice.

Qualifications

• Active Florida Physical Therapist Assistant license or eligible to practice in Florida.
• Experience in home health or geriatric care preferred.
• Strong interpersonal and time management skills.
• Ability to treat patients independently in home settings.
• Ability to complete documentation accurately and on time.

Responsibilities

• Treat patients in their homes throughout Palm Beach County.
• Provide individualized treatment sessions based on the patient’s plan of care.
• Assist patients with improving strength, balance, mobility, safety, and independence.
• Engage family members and caregivers as needed.
• Provide patient education and home exercise program instruction, plus DME recommendations when appropriate.
• Complete timely and accurate documentation through EMR.
• Maintain frequent communication with supervising therapist and office team.
• Follow Medicare and home health documentation guidelines.
