You will provide in-home PT/PTA visits, complete initial evaluations, reassessments, and fall-risk assessments, and communicate progress with the care team to keep seniors safe at home.

Qualifications

• Current Hawaii license as PT or PTA.
• 1+ year of clinical experience (home health preferred).
• Strong documentation and communication skills.
