Job summary

FOX Rehabilitation is seeking licensed Physical Therapists for in-home, senior living, and telehealth work in Toms River, NJ. You’ll deliver one-on-one therapy to older adults, evaluate and plan care, and help patients gain mobility and independence.

This role emphasizes autonomy, flexible scheduling, and meaningful patient outcomes within a clinician-driven team. Telehealth may supplement in-person care where appropriate.

Qualifications

• Valid Physical Therapy license in the state of practice or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• CPR certification and basic computer/EMR proficiency.

Responsibilities

• Provide one-on-one physical therapy to older adults in home or community settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, mobility, and independence.
• Document in FOX’s EMR system in line with regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate to extend access.
