Job summary

InHome Therapy is seeking licensed physical therapists to provide in-home care for adult and geriatric patients in Libertyville and surrounding communities. The role offers flexible scheduling, a consistent caseload, mileage reimbursement, and provided tablet, data plan, and supplies.

Compensation is per visit with a length of stay per visit at $65–$85, plus benefits and paid training. PTO, holidays, and mentorship are included, with opportunities for leadership and professional growth.

Qualifications

• Active PT license in good standing
• BLS CPR certification
• Reliable transportation to see patients in their homes
• Comfort with technology (email, texting, EMR systems – training provided)

Responsibilities

• Deliver in-home physical therapy services to adult and geriatric patients
• Evaluate mobility, strength, and function to develop individualized treatment goals
• Implement rehabilitation and therapeutic exercise programs
• Educate patients and families on recovery plans and preventive care
• Maintain accurate and timely home health care documentation per Medicare standards
