• Performs age-appropriate PT care in the home setting including admission visits, evaluation, and treatment.
• Adheres to system standards, conducts visits promptly and accurately.
• Focuses on customer service and patient satisfaction.
• Develops and participates in process improvements within the home health agency.
• Supervises the Physical Therapy Assistant according to State Board guidelines.
• Communicates with family, physicians, and the care team to ensure optimal patient care.
• Maintains documentation including visit assessments and orders in a timely manner.
