• Performs duties and responsibilities in accordance with system-wide competencies/behaviors.
• Attends compliance training and adheres to organizational standards, policies, and procedures.
• Delivers age-appropriate home health physical therapy care, including admission visits, evaluation, instruction, and treatment.
• Maintains timely and accurate documentation (visit notes, OASIS, 485, verbal orders) on laptop or written records.
• Collaborates with physicians, family, and interdisciplinary team to optimize patient care.
• Participates in on-call coverage rotation for the agency.
• Supervises Physical Therapy Assistant per State Board guidelines.
