Job summary

The ideal candidate holds a Kentucky nursing license (LPN or RN), CPR certification, and TB test, with willingness to obtain if needed. On-call coverage and strict documentation are required.

Qualifications

• Kentucky nursing license (LPN or RN) is required.
• CPR certification or willingness to obtain.
• TB skin test or willingness to obtain.
• Competent in general nursing skills and practice.
• Private duty experience helpful, but not required.

Responsibilities

• Provide skilled nursing care in the home per care plan and physician orders.
• Document visits and progress notes in a timely manner.
• Educate patients on health maintenance and self-care techniques.
• Monitor and report patient status to care team.
• Follow infection control and safety policies.
• Maintain confidentiality of patient records.
• Participate in on-call schedule to cover after-hours needs.
