Job summary

Well Care Health is seeking a compassionate Home Health Physical Therapist Assistant to provide in-home physical therapy and related care to patients across all life stages. You will work under the supervision of the Clinical Manager and a licensed physical therapist, delivering individualized patient care in patients’ homes.

The role requires an Associate Degree as a PTA, state licensure (NC/SC), and CPR certification.

Qualifications

• Associate degree from an accredited PTA program.
• Licensure in the state where therapy is provided (NC/SC).
• CPR certification is required.

Responsibilities

• Provide in-home physical therapy and related care with patients across life stages.
• Monitor patient status, report changes, and assist in care plan development.
• Educate patients and families on health management, medications, and self-care techniques.
• Collaborate with the healthcare team to ensure high-quality patient care.
• Administer prescribed physical therapy treatments in all life stages.
• Assist with program development and discipline-specific operations.
