Job summary

CenterWell Home Health is seeking a Home Health Physical Therapy Assistant to help patients regain strength and independence in their homes, under the supervision of a Physical Therapist and Clinical Manager.

You will deliver treatment, perform therapeutic exercises and modalities, document visits weekly, and participate in team in-services, with TB and driver safety policies in place.

Qualifications

• Current state PTA license to practice.
• Valid driver's license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Comprehensive knowledge of geriatric patient changes.

Responsibilities

• Deliver PT treatment plan to patients in home health settings as developed by the Physical Therapist.
• Perform non-complex active and passive manual therapy, therapeutic exercises, massage and related modalities.
• Document client records and submit weekly clinical documentation.
• Attend educational meetings and coordinate department in-services.
• Notify supervisor of equipment failures, supply issues, or staff or client changes.
• Apply infection control and universal precautions in patient care.
