Job summary

CenterWell Home Health is seeking a dedicated Physical Therapist Assistant to support patients in their homes. You will work under a Physical Therapist to implement treatment plans focused on restoring strength, mobility and independence, while documenting progress and coordinating care with the clinical team.

The role requires an active PTA license, valid driver's license, CPR certification, and strong teamwork. English fluency and geriatric care knowledge are essential for success.

Qualifications

• Current state license to practice Physical Therapy.
• Valid driver‘s license and reliable transportation.
• Current CPR certification.
• Experience collaborating with a healthcare team.
• Geriatric population knowledge (physical, emotional, social, biological).
• Fluent reading, writing and speaking English.

Responsibilities

• Administer the physical therapy treatment plan in patients' homes, as developed by the Physical Therapist.
• Perform non-complex active and passive therapeutic exercises, massage, traction, heat, cold, water and electrical modalities as directed.
• Document client records and submit weekly clinical documentation.
• Attend educational meetings and coordinate department in-services.
• Notify supervisor of equipment failures, supply issues, or staff changes.
• Apply infection control and universal precautions in patient care.
