Job summary

CenterWell Home Health, a part of Humana Inc., is seeking a Home Health Physical Therapist Assistant to deliver patient-centered care in patients' homes. You will implement the PT plan, guide exercises, and document progress while coordinating with the PT and branch leadership.

Required licenses and certifications include a current PT license, valid driver's license, CPR, and ability to work within a healthcare team. Prior geriatric care experience is preferred.

Qualifications

Responsibilities

• Administer PT treatment plan onsite in patients' homes as developed by the PT.

• Document client records and submit to the office weekly.
• Attend educational meetings and coordinate department in-services.
• Notify PT or Branch Director of equipment or staff changes.
• Apply infection control and universal precautions in patient care.
