Job summary

CenterWell Home Health is seeking a dedicated Physical Therapist Assistant to provide in-home care under the supervision of a Physical Therapist. You will implement the treatment plan, administer exercises and modalities, and document patient progress to help regain function and independence.

Qualifications

• Current state license to practice Physical Therapy.
• Valid driver’s license with reliable transportation.
• Current CPR certification.
• Experience collaborating with a healthcare team.
• Geriatric care knowledge and English fluency.

Responsibilities

• Administer the physical therapy treatment plan to patients at home as developed by the Physical Therapist.
• Perform non-complex manual therapy, massages, and modalities as prescribed.
• Document client records and submit weekly clinic documentation.
• Attend educational meetings and coordinate department in-services.
• Notify PT or Branch Director of equipment issues or staff changes.
• Apply infection control and universal precautions during care.
