Job summary

CHI Health Connect Home-Fargo is seeking a Physical Therapy Assistant to deliver compassionate, personalized home-based care under the direction of a Physical Therapist and a physician's plan of care. You will help patients recover from injuries, manage chronic conditions, and support families in achieving optimal functional outcomes.

Qualifications

• Graduate of an accredited physical therapy assistant program with an Associate's degree in science or applied science approved by the American Physical Therapy Association.
• Licensed to practice as a Physical Therapy Assistant within the ND and MN.
• Possesses and maintains current CPR certification.
• Current Driver’s license.
• Auto Insurance.

Responsibilities

• Provide direct physical therapy under the direction of the Physical Therapist and according to the physician's plan of care.
• Assist patients with active and passive therapeutic exercises, gait and functional training, transfers and prosthetic/orthotic training.
• Observe and report patient responses to treatment to the Physical Therapist.
• Instruct patients in use of wheelchairs, braces, crutches, canes, and orthotic devices.
• Maintain and update clinical notes and records within 24 hours of patient care.
• Participate in Home Care team meetings and in-service education programs.
