Job summary

CenterWell Home Health is seeking a Home Health Physical Therapy Assistant to administer and assist with the PT plan in patients' homes. You will perform non-complex modalities, document visits, and coordinate in-services while upholding infection control standards.

The role involves driving to patient homes, maintaining licensure, and joining a team dedicated to geriatric care, with a $5k sign-on bonus for eligible candidates.

Qualifications

• Active state license to practice Physical Therapy.
• CPR certification required.
• Experience collaborating with healthcare professionals.
• Knowledge of geriatric physical changes.

Responsibilities

• Administer the physical therapy treatment plan to patients in their homes as developed by the Physical Therapist.
• Administer non-complex active and passive manual therapies, therapeutic massage, traction, heat, light, cold, water, and electrical modalities.
• Document client records and submit clinical documentation on time each week.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment failures, supply issues, or staff/client changes.
• Apply infection control and universal precautions in patient care.
