Job summary

CenterWell Home Health, a Humana company, is seeking a Home Health Physical Therapist Assistant to perform patient-focused PT services in patients' homes, under supervision of a Physical Therapist and reporting to the Clinical Manager.

The role requires current PT license, CPR, a valid driver's license, and the ability to travel to patients' homes. Pay per visit ranges from $41 to $57, and annual pay range is $64,000–$87,500 with benefits described in the Humana program.

Qualifications

• Current state license to practice Physical Therapy.
• Valid driver's license with auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a healthcare team.
• Knowledge of geriatric physical, emotional, social, and biological changes.
• Read, write and speak fluent English.

Responsibilities

• Administer the physical therapy treatment plan to patients in their homes.
• Administer non-complex manual therapies and modalities as directed.
• Document client records and submit clinical documentation on time.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment or staff changes.
• Apply infection control and universal precautions in patient care.
• Use your skills to make an impact.
