Job summary

FOX Rehabilitation in Cranford, NJ is seeking licensed Physical Therapists to provide one-on-one, in-home care to older adults across private residences and senior living communities. You will evaluate patients, develop plans of care, and deliver evidence-based therapy that promotes independence and mobility, with the flexibility to manage your own schedule and telehealth options where appropriate.

Qualifications

• Must hold valid Physical Therapy license in the state of practice.
• CPR certification required.
• Proficiency using EMR systems preferred.

Responsibilities

• Provide one-on-one physical therapy to older adults in-home or in senior living settings.
• Perform evaluations and develop individualized plans of care.
• Deliver therapy sessions to improve mobility and independence.
• Document patient care accurately in FOX EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Offer telehealth/virtual care when appropriate to extend access.
