Job summary

Body Rehab Solutions in Santa Rosa, California is seeking a dedicated Physical Therapist to join our home-based care team as a Full-Time clinician. You will deliver one-on-one therapy in patients' homes, develop individualized treatment plans, educate families, and document progress while collaborating with physicians and a multidisciplinary team.

Qualifications

• Current and valid state Physical Therapy License.
• Bachelor’s degree in Physical Therapy, with preference for Doctor of Physical Therapy (DPT).
• Basic Cardiac Life Support (BCLS) certification, or the ability to obtain this certification within 30 days of employment.
• No prior experience is necessary; we welcome new graduates eager to make a positive impact in patient care.

Responsibilities

• Conduct comprehensive assessments of each patient’s physical abilities and limitations to develop tailored rehabilitation plans in collaboration with physicians and the multidisciplinary care team.
• Implement and oversee individualized therapy programs, adapting techniques as necessary to optimize each patient’s recovery process.
• Deliver hands-on therapeutic care directly in patients’ homes, employing advanced physical therapy modalities and exercises designed to enhance physical function and independence.
• Educate patients, families, and caregivers about therapy goals and methods, ensuring they are confident in executing prescribed home exercises and utilizing necessary adaptive equipment.
• Maintain accurate and detailed documentation of patient progress, modifications to treatment plans, and communicate with the care team to ensure coordinated, high-quality care delivery.
• Manage and prioritize your caseload effectively, ensuring timely and efficient patient care while maintaining compliance with relevant laws and regulations.

Education

Bachelor’s degree in Physical Therapy

Doctor of Physical Therapy (DPT) preferred
