Job summary

Rosaires Care is seeking a licensed physical therapist to provide skilled therapy in patients’ homes. You will assess and treat individuals, educate families, and ensure safe mobility and independence across a broad service area.

The role involves collaborating with interdisciplinary teams, documenting progress, and delivering high-quality care in home settings, with travel to patient residences as needed. Performance is guided by patient-centered outcomes.

Qualifications

• Must be a licensed physical therapist.
• Experience in home-care settings preferred.
• Willingness to travel across a designated geographic area.
• Strong communication with patients and caregivers.

Responsibilities

• Assess patients’ condition, functional abilities, and needs.
• Develop, implement, and adjust individualized treatment plans.
• Coordinate care with interdisciplinary teams to support patient outcomes.
• Educate patients and caregivers on exercises, mobility strategies, and safety.
• Document patient assessments, interventions, and progress according to required standards.
• Deliver therapy services across a designated geographic area with travel to patients’ residences.
