Qualifications

• Must have a current and unrestricted Physical Therapy license.
• Current CPR certification required.
• Experience working as part of a healthcare team.

Responsibilities

• Plan and administer prescribed physical therapy treatments at patient homes.
• Assess and document patients' physical strengths to guide treatment.
• Develop and implement rehabilitation programs according to physician’s guidelines.
• Provide clear instructions to PTAs and Home Health Aides.
• Document patient observations and report progress to the healthcare team.
