Responsibilities

• Plan and administer prescribed physical therapy treatment for patients, working onsite in their homes.
• Assess and document patients' physical strengths to support physician evaluation.
• Develop and implement rehabilitation programs aligned with physician plans and patient goals.
• Provide PTAs and Home Health Aides with clear care instructions and supervise their documentation.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report patient progress to physician, Clinical Manager and care team, and submit required documents.
