Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist to plan and administer prescribed physical therapy in patients' homes, aiming to restore function, mobility and independence through personalized care. With a 40-hour work week, you will collaborate with physicians and care teams, document progress, and guide PTA and home health aides.

The role requires an active PT license, CPR certification, and a commitment to high-quality patient care.

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Responsibilities

• Plan and administer prescribed physical therapy treatment in patients' homes.
• Assess and document patients' physical strengths to guide goals.
• Develop and implement rehab programs aligned with physician plans.
• Provide PTAs and Home Health Aides with clear written instructions and oversee performance.
• Document observations, interventions, and evaluations promptly.
• Report patient progress to physicians, care team, patient and family, including required notes.
• Use your skills to make an impact.
