Job summary

Adventist Health in Bay City, OR is seeking a Physical Therapist to plan and conduct segments of treatment programs aimed at restoring and improving physical, social and mental functions. You will act as a co-leader in an interdisciplinary care approach.

The role requires a Doctor of Physical Therapy (DPT), state licensure, CPR/BLS certification, and a valid driver’s license (21+). You will evaluate patients, develop individualized care plans, document comprehensively, and supervise assistants

Qualifications

• Doctor of Physical Therapy degree required; only if license obtained on/after Jan 2016; or foreign equivalent.
• Licensure as Physical Therapist in all applicable states: required.
• CPR or BLS certification from AH policy: required.
• Valid driver's license and must be 21+ years old: required.

Responsibilities

• Completes evaluations to inform quality standards and plans.
• Provides therapy interventions using approved protocols and discharges plans.
• Completes comprehensive clinical documentation within deadlines.
• Supervises assigned assistants; performs other duties as assigned.
