Job summary

Home Health Care, Inc. is seeking a Community-Based Physical Therapist (PT) to deliver in-home therapy across Minnesota. The role offers full-time hours, a Monday–Friday schedule, and compensation ranging from six figures plus a retention bonus.

Qualifications

• Active Minnesota Physical Therapist (PT) license.
• Excellent clinical reasoning and communication skills.
• Passion for helping patients achieve meaningful, lasting outcomes.
• Ability to work independently in a field-based setting.
• Valid driver's license and reliable transportation.

Responsibilities

• Evaluate patients in their homes and create individualized treatment plans.
• Improve mobility, strength, balance, endurance, and functional independence.
• Educate patients and caregivers to support long-term recovery.
• Collaborate with nurses, physicians, occupational therapists, speech therapists, and other members of the interdisciplinary team.
• Monitor progress and adjust treatment plans to maximize outcomes.
• Complete accurate and timely documentation.
