Job summary

Select Home Health in Bloomington, IN is seeking a compassionate Physical Therapist to provide skilled in-home therapy, developing individualized care plans to improve function and safety.

You will evaluate patients, treat using modalities, document progress, and coordinate with physicians, DON, and care team. Indiana license and 1 year experience required; graduate of ACPT program. In-home visits with comprehensive benefits and PTO.

Qualifications

• Graduate of an accredited physical therapy program (APTA or AMA approved).
• Licensed as a Physical Therapist in the state of Indiana.
• Minimum of 1 year of experience as a Physical Therapist.

Responsibilities

• Provide skilled physical therapy in patients' homes according to the physician's plan of care, seeing multiple patients per day.
• Evaluate and assess patient function and assist in developing and updating individualized care plans.
• Deliver treatments to relieve pain, restore function, and support maximum performance using appropriate modalities.
• Monitor patient progress, document outcomes, and communicate changes to the physician, DON, and care team.
• Educate patients, caregivers, and staff on therapy goals, techniques, and use of adaptive devices or equipment.
• Reassess patient needs regularly and revise plans of care as appropriate.
