Job summary

LiveWell Partners is seeking a Home Health Physical Therapist (PT) to evaluate patients in the home health setting and develop, implement, and oversee comprehensive care plans. The PT collaborates with an interdisciplinary team to deliver therapy services and ensures proper documentation and progression of care.

Qualifications

• Minimum of 1 year of physical therapy experience (acute or post-acute settings preferred).
• CPR certification is required.
• Active license to practice physical therapy in the state.

Responsibilities

• Determine physical therapy treatment plans in consultation with care team and physicians through initial and ongoing assessments.
• Provide and complete all proper documentation.
• Administer physical therapy treatments as ordered by a physician; direct and oversee assistants.
• Evaluate progress and adjust therapies; assess prosthetic/orthotic device fit.
• Complete discharge planning and coordinate post-discharge continuation of therapy.
• Participate in weekly case conferences and communicate across disciplines.
