Job summary

Canyon Idaho Home Care and Hospice in Fruitland, ID seeks a Physical Therapist to administer home-based therapy under physician orders. You will work under supervision of Clinical Manager/Therapy Director to evaluate patients, develop plans of care, and coordinate with families and care teams.

Responsibilities include supervising PTA/HHA staff, completing assessments, and documenting patient progress per agency timelines. Licensure is required and experience in home health is beneficial.

Qualifications

• Licensed physical therapist as applicable by state.
• Graduated from CAPTE-accredited PT program or equivalent.
• Passed licensure/exam requirements for the practicing state.

Responsibilities

• Assists physician or intermediate care provider in evaluating level of function and develops revised plan of care.
• Advises and consults with family and other agency personnel.
• Participates in in-service programs and presents as assigned.
• Adheres to established policies and procedures.
• Coordinates with all health care personnel in patient care planning.
• Directs and supervises PTA and HHA per requirements.
• Completes initial history, notifies physician, and submits plan of care for approval.
• Performs skilled PT procedures as ordered by physician.
• Writes reports to physicians regarding progress.
• Instructs patients and families in home programs and daily living activities.
