Job summary

Valley Health in the United States is seeking a Home Health Physical Therapist to deliver direct patient care in patients' homes as part of a multidisciplinary team. You will evaluate patients, design individualized plans of care, and supervise assistants while documenting in an electronic record. A 4–6 week orientation with a seasoned preceptor helps you prepare.

The role requires a PT degree, state licensure/eligibility, and BLS, with opportunities to impact daily lives across varied ages.

Qualifications

• Bachelors, master's or doctoral degree in Physical Therapy is required.
• A completed CI credentialing course, home study adult learning course, or equivalent, is required.
• Minimum two years of clinical experience (4,160 hours) is required.
• Physical Therapy license or eligibility in the state of assignment is required.
• BLS certification (American Heart Healthcare Provider) required before orientation.
• Valid driver's license and reliable transportation required.
• Basic computer and keyboarding skills required.
• Must be able to transfer electronic data.
• Must not have barrier crimes.

Responsibilities

• Provide direct, one-on-one patient care in the home environment.
• Evaluate patients and design individualized plans of care with a multidisciplinary team.
• Supervise care provided by assistants.
