Job summary

Human Touch Home Health seeks a full-time Physical Therapist to provide in-home care across Blacksburg, Christiansburg, Radford and nearby communities. You’ll perform evaluations, develop plans of care, guide patients through therapeutic exercises, assess safety, and educate families.

Qualifications

• Active Virginia Physical Therapist license required.
• Graduate of an accredited Physical Therapy program.
• Valid Driver's License and reliable transportation.
• Strong clinical and communication skills.
• Ability to work independently in patients' homes.
• Home Health/OASIS experience preferred, not required.
• WellSky experience is a plus.

Responsibilities

• Complete physical therapy evaluations in patients' homes.
• Develop individualized plans of care.
• Provide therapeutic exercise, gait, balance, transfer and mobility training.
• Assess home safety and fall risks.
• Educate patients and caregivers.
• Monitor patient progress and adjust treatment plans.
• Complete progress assessments and discharge planning.
• Coordinate with physicians, nurses, PTAs, and other care team members.
• Complete accurate documentation using WellSky.
