Job summary

Well Care Health is seeking a compassionate Home Health Physical Therapist to deliver in-home care focused on patient education, safe mobility, and functional outcomes. You will assess patients, develop individualized plans, and provide evidence-based PT interventions in patients’ homes.

Requirements include a graduate PT degree, active NC or SC licensure, CPR certification, and at least 1 year of experience.

Qualifications

Graduate of Physical Therapy program from an accredited university.
Licensure in NC/SC; CPR certification.
Minimum 1 year of experience in physical therapy or home health.

Responsibilities

Assess and develop plans for patients in their homes.
Provide PT services including therapeutic exercises, modalities, and gait training.
Educate patients and families on home exercise programs and safety.
Document patient progress and communicate with the care team.
Coordinate with clinical manager and other professionals.
Maintain schedules and ensure efficient service delivery.
Demonstrate commitment to patient satisfaction and quality care.
Uphold agency values and comply with policies and procedures.
Assist in program development and performance improvement initiatives.
Support safety and infection control guidelines.
Travel to patient homes and manage transportation as required.
