Job summary

Banner Health is seeking a Physical Therapist to join Banner Home Health in the Greeley area. You will evaluate, plan, administer, and supervise PT services for home-bound patients, collaborating with an interdisciplinary team to provide patient-centered care.

Locally located in Greeley, CO—located about 45 minutes north of Denver—this role offers independent scheduling, with a typical 32-hour weekend-based schedule and potential additional weekday days.

Qualifications

• Graduate from an APTA accredited school of Physical Therapy.
• Current Physical Therapy license in the state of practice.
• BLS certification required.
• Ability to become proficient with computer software for patient care documentation.

Responsibilities

• Evaluate patient functional abilities and establish a plan of care with measurable goals.
• Implement individualized treatment plans and provide skilled therapy.
• Document progress, goals, and modalities in the chart.
• Communicate patient progress to physicians, nurses, and the care team.
• Supervise PTA, ATC, Therapy Technician and students per department policy.
