Job summary

Pure Home Health Care in Michigan is seeking a full-time Physical Therapist to deliver in-home care to patients in their residences. You will provide PT services, develop home exercise plans, and document patient progress according to physician orders.

Avalid Michigan PT license and a current Michigan Driver's license with reliable transportation are required; two years of PT experience is preferred. Flexible scheduling including evenings and weekends is expected.

Qualifications

• Must hold a Michigan Physical Therapist license and graduate from an approved PT program.
• Two years of PT experience preferred and must meet licensure requirements.
• Current Michigan Operator's License and reliable transportation required.
• Be willing to work flexible hours including evenings and weekends as needed.

Responsibilities

• Provide in-home physical therapy services per physician plan of care.
• Assess functional status and activities of daily living.
• Develop and supervise home exercise programs and document progress within 48 hours of visit.
• Reassess patient needs and adjust plans of care as required.
