Job summary

FOX Rehabilitation is seeking a licensed Physical Therapist to provide in-home and senior living care in Norfolk, VA. You will evaluate patients, create individualized care plans, and deliver evidence-based PT sessions in private residences or communities.

Role emphasizes autonomy, flexible scheduling, and meaningful patient outcomes. Telehealth components and interdisciplinary collaboration are available to extend access and support more patients.

Qualifications

• Valid Physical Therapy license in the state(s) of practice or eligibility to obtain
• Experience with geriatric populations and home-based care preferred
• Ability to document accurately in EMR and coordinate with an interdisciplinary team

Responsibilities

• Provide one-on-one PT to older adults in private residences or senior living settings
• Develop and implement individualized care plans and evidence-based treatments
• Deliver therapy sessions to improve mobility, independence and safety
• Educate patients, caregivers and facility staff to promote safety and aging in place
• Incorporate telehealth or virtual care when appropriate to extend access
