Job summary

FOX Rehabilitation is growing in Ramsey, NJ, and seeks licensed Physical Therapists to deliver one-on-one care in patients\' homes or senior living communities. You\'ll design individualized plans and treat older adults, with an emphasis on mobility, independence, and quality of life.

Clinicians may use telehealth and other virtual care modalities to extend access, while maintaining autonomy over their schedule and patient relationships within a supportive, interdisciplinary team.

Qualifications

• Licensed PT in NJ or eligible to obtain license
• DPT or equivalent from accredited PT program
• Basic computer and EMR proficiency
• CPR certification required

Responsibilities

• Provide one-on-one PT to older adults in home or senior living settings
• Perform evaluations, develop plans of care, deliver evidence-based treatment
• Deliver sessions to improve wellness, mobility and independence
• Document in FOX EMR per regulatory standards
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff on safety and aging in place
• Provide telehealth/virtual care when appropriate
