FOX Rehabilitation is growing in Selinsgrove, PA, and seeks licensed Physical Therapists to deliver one-on-one geriatric care in patients' homes or senior living communities. You will evaluate, develop plans of care, and deliver evidence-based therapy while building strong patient relationships and controlling your own schedule.

Qualifications

• Licensed Physical Therapist with active license in the relevant state.
• Degree from an accredited Physical Therapy program.
• Proficient with EMR and basic computer skills.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver sessions that promote mobility, independence and function.
• Document care in FOX's EMR per regulatory standards.
• Collaborate with an interdisciplinary PT/OT/SLP team.
• Educate patients, caregivers, and staff to promote aging in place.
• Provide telehealth/virtual care when appropriate.
