Job summary

FOX Rehabilitation in West Roxbury, MA, is seeking licensed Physical Therapists to deliver one-on-one care to older adults in homes or Senior Living Communities under the Geriatric House Calls model. You’ll evaluate, plan care, and provide evidence-based therapy, with telehealth options and flexible schedules, collaborating with an interdisciplinary team to promote independence and aging in place.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in home or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to improve wellness, mobility, and independence.
• Document in FOX’s EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
