Job summary

In Home Rehab Group is seeking a compassionate Physical Therapist to join our Broward County team in a part-time/PRN role. You will perform in-home evaluations and treatments, delivering one-on-one care to older adults across Broward, with flexible hours and steady opportunities through our agency contracts.

Active Florida PT license required; prior home health or geriatric experience preferred. You will collaborate with the care team, educate patients and families, and document visits in EMR.

Qualifications

• Experience delivering in-home PT care to older adults.
• Ability to develop and adjust treatment plans.
• Strong documentation and EMR skills.

Responsibilities

• Perform in-home PT evaluations and re-evaluations.
• Treat patients one-on-one in home settings across Broward County.
• Create individualized treatment plans focused on strength, mobility, safety, and independence.
• Provide patient education and home exercise program instruction.
• Document visits timely in EMR and follow guidelines.
