Responsibilities

• Perform PT evaluations, re-evaluations, progress notes, and discharge visits.
• Treat patients in their homes across Broward County.
• Create individualized treatment plans based on patient needs and goals.
• Provide one-on-one care focused on improving strength, balance, mobility, safety, and independence.
• Supervise and communicate with PTAs as needed.
• Engage family members and caregivers when appropriate.
• Provide patient education, home exercise program instruction, and DME recommendations.
• Complete timely and accurate documentation through EMR.
• Follow Medicare and home health documentation guidelines.
• Maintain frequent communication with the office team, patient representatives, and other care team members.

Physical Therapist - Part-time/PRN- Broward County

In Home Rehab Group provides in-home therapy services to older adults throughout South Florida. We work with Medicare Part A and Part B patients and partner with multiple home health agencies, allowing our therapists to access consistent opportunities through one company instead of having to contract with several agencies on their own.

We are looking for an experienced, dependable, and compassionate Physical Therapist to join our growing team in Broward County. This is a great opportunity for a PT who enjoys treating patients one-on-one in the home setting, wants flexibility, and is looking to add consistent visits to their schedule.

At In Home Rehab of Florida, we do the heavy lifting of finding contracts and building agency relationships so you can focus on what you do best: treating patients. Responsibilities:

Requirements:
