Job summary

Metro Physical & Aquatic Therapy is seeking a motivated full-time Physical Therapist to provide in-home care. The role requires travel to patients' homes within your geographic area and offers growth within a supportive, innovative team environment.

All levels of experience are encouraged to apply; mentorship is available for new graduates. The anticipated base salary range is $95,000–$110,000 with full benefits, generous PTO, and ongoing professional development opportunities.

Qualifications

• Licensed Physical Therapist in NY or eligible.
• Physical Therapist Program Graduate.
• Home health experience is preferred but not required.

Responsibilities

• Accept and treat patients within the designated geographic area.
• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans.
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family.
• Plan, prepare and implement customized therapy programs to restore function and promote independence.
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning.
• Deliver care in compliance with agency policies and professional practice standards.
