Job summary

Iowa Home Care LLC is seeking a licensed Physical Therapist to provide in-home evaluations and individualized treatment plans, helping patients improve mobility, strength, balance, and safety in the comfort of their own homes. You will collaborate with an interdisciplinary team, educate patients and caregivers, document progress, and recommend equipment or home modifications as needed.

A valid Iowa license, at least two years of PT experience (home health preferred), and reliable transportation

Qualifications

• Licensed to practice as a Physical Therapist in the state of Iowa.
• Minimum of two years of experience in physical therapy; community/home health experience preferred.
• Valid driver’s license with reliable transportation.

Responsibilities

• Conduct comprehensive in-home physical therapy evaluations.
• Develop and implement individualized treatment plans focused on mobility, strength, balance, and safety.
• Provide therapeutic interventions including exercise programs, gait training, transfer training, and pain management.
• Educate patients and caregivers on home exercise programs, safety strategies, and equipment use.
• Collaborate with nurses, occupational therapists, speech therapists, social workers, and case managers to coordinate care.
• Document all patient visits, progress notes, and care plans in compliance with agency and regulatory standards.
• Recommend adaptive equipment and home modifications when appropriate.
• Monitor patient progress and adjust treatment plans as needed.
