Job summary

InHome Therapy is seeking licensed home health Physical Therapists to serve adult and geriatric patients in their homes around Hoffman Estates, IL. You’ll manage a flexible schedule with a stable caseload, supported by our agency network and clinical leadership.

Deliver in-home PT services, evaluate mobility and function, implement rehab programs, and educate families on recovery plans. Per-visit pay ranges $65–$85, with benefits such as PTO, holidays, and ongoing training.

Qualifications

• Active physical therapy license in good standing.
• BLS CPR certification required.
• Reliable transportation to visit patients in homes.
• Comfort with technology; EMR training provided.

Responsibilities

• Deliver in-home physical therapy services to adult and geriatric patients.
• Evaluate mobility, strength, and function to set goals.
• Implement rehabilitation and therapeutic exercise programs.
• Educate patients and families on recovery plans and preventive care.
• Maintain accurate home health care documentation per Medicare and agency standards.
