Job summary

Fox Rehabilitation in Greenwood, IN is seeking a part-time Physical Therapist to deliver one-on-one geriatric care in patients’ homes or senior living communities. You’ll conduct evaluations, plan individualized care, and provide evidence-based therapies designed to improve mobility and independence.

You will collaborate with a multidisciplinary team and may incorporate telehealth to extend access and continuity of care.

Qualifications

• Valid Physical Therapy license in the state(s) of practice or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in-home or in senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, mobility and independence.
• Document effectively within FOX’s EMR system in line with regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
